Job Title: Senior Care Co-Ordinator / Office Manager
Reports To: Registered Manager

Senior Care Coordinators may be the first point of contact for patients and are primarily responsible for arranging and supervising interdisciplinary and individual patient care.
A Senior Care Coordinator will work closely with the service user to initially assess their needs, in order to develop and deliver personalised – and often complex – care plans. This is a very varied and busy role.

Reports to;
• Registered Manager.
This role may involve:

• Meeting with patients, as well as any relevant family members or carers, to discuss what they need and want from their care, and what services are available to them to support their personalised care requirements, sign posting and navigation.
• Developing a single personalised care plan, as well as monitoring and adjusting them when needed, to always ensure seamless provision of relevant services in line with best practice.
• Visiting, and maintaining excellent communication levels with, patients to check on the care they are receiving and keeping relevant clear documentation.
• Evaluating interventions and care plans with the care team, including identifying areas where and when further care may be required.
• Supporting patients on an ongoing basis to manage their own needs where possible, promote and encouraging patient awareness, skills and confidence to them help maintain, and make decisions about their own health and wellbeing, including personal health budgets
• Maintaining a good awareness of developments in the field.
• 3 years experience as a care coordinator
• Performing spot checks on staff.
• Review Medication for each service user where Addi care services Ltd is responsible for administering Medication and ensure the manual and electronic MAR charts are in sync.
• Ensure new staff are assigned online and Face to Face Training.
• Maintain the Training Matrix.
• Perform monthly medication audits.
o Participate in audits.
o Bi-Weekly spot checks on staff.
o Monthly visits to clients to discuss their care and get their feedback.
o Audit Staff Files.
o Monthly Staff meetings
• Obtain Service User & Employee Feedback on a bi-annual basis.
• Ensure staff have access to Policies & Procedures and that they have read and affirmed the mandatory key Policies and Procedures on the QCS app.
